Hospice Patient Navigator
Job Description
Passages Hospice is seeking a Hospice Patient Navigator to support referral coordination and strengthen connections across the healthcare community. The role is onsite in Alexandria, LA and centers on maintaining continuity of care through clear communication, accurate documentation, and patient-centered coordination.
Role Overview
This position coordinates patient referrals and collaborates with local hospitals to secure referrals. In addition, the Navigator delivers community education about hospice care and works directly with patients and their care teams to ensure continuity of care, effective communication, and correct documentation.
Key Responsibilities
- Coordinate patient referrals
- Develop relationships with local hospitals to secure referrals
- Provide education for the community and referral sources on hospice care
- Collaborate with patients and their care team to support continuity of care, proper communication, and accurate documentation
Required Qualifications
- 2 years of required experience in healthcare marketing
- Bachelor’s degree in Marketing, Communications, or a related field preferred; equivalent work experience considered
- Proven experience developing marketing strategies
- Excellent written and verbal communication skills
- Strong project management and organizational skills
- Experience in event planning and public speaking
- Ability to work independently and as part of a team
- Compassion and empathy for patients and their families
Preferred Qualifications
- Familiarity with medical terminology and the healthcare industry
- Willingness to travel (preferred)
Compensation & Schedule
- Job type: Full-time
- Salary: From USD 75,000 per year
- Work location: In person (Alexandria, LA)
- Travel: 75% (preferred)
