Job Description

This director-level position oversees hospital referral relationships, census growth, and market positioning for a Skilled Nursing Facility in Tucson. It combines field-based leadership across external sales, referral development, and local market marketing strategy.

Responsibilities

  • Serve as the primary external liaison to hospitals, collaborating with case managers, discharge planners, social workers, and physicians to coordinate patient transitions
  • Actively identify patients moving toward post-acute care after discharge planning
  • Lead admissions for both short-term rehabilitation and long-term care
  • Own the end-to-end referral process from initial hospital identification through patient admission
  • Present the facility’s clinical capabilities and outcomes to secure placements
  • Manage Medicare, managed care, and authorization workflows
  • Monitor and report on referral volume, conversion rates, payer mix, and census impact
  • Identify and cultivate new hospital and referral partnerships
  • Develop and execute local market marketing plans to support census growth
  • Enhance the facility’s brand presence with hospitals and community partners
  • Determine competitive differentiators and communicate them effectively
  • Coordinate outreach efforts, events, and targeted referral campaigns
  • Provide market intelligence and competitive insights to leadership
  • Collaborate with leadership to align service lines with market demand
  • Work closely with the Administrator, Director of Nursing, Therapy, and Admissions teams
  • Ensure a smooth handoff from referral to admission
  • Identify and remove internal barriers that delay conversions
  • Ensure all sales and marketing activities comply with CMS and state regulations

Requirements

  • At least five years of SNF or post-acute clinical liaison and marketing/sales experience
  • Proven success in driving hospital referrals and census growth
  • Strong understanding of hospital discharge workflows
  • Experience with Medicare and Managed Care authorizations
  • Relationship-driven with a solid sales mindset
  • Existing hospital relationships are preferred
  • Valid driver’s license and the ability to travel locally
